A healthcare organization in California is actively seeking a Social Worker (MSW) to join their growing team at one of their group homes in Santa Cruz County.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: Ongoing (Contract to Hire)
- Schedule: Part Time
- Hours: Tuesdays & Thursdays (12pm to 5pm) and Fridays (8am to 12pm)
- Patient Population: Adults and Families with mental health and substance abuse challenges
Responsibilities:
- Assist in the completion of assessments and plans for each person in the home
- Obtain, develop, and record the information necessary for the completion in person or remotely
- Ensure compliance with all applicable regulations and timelines
- Complete intake assessments during the referral process
- Review current and historical records and documentation
- Perform direct observations and interviews with the individual and their support group to determine support needs and services required for success
- Participate in the transitional planning process working closely with the Qualified Behavior Modification Professional (QBMP), including Individual Education Plan (IEP), Individual Program Plan (IPP), and Individual Behavior Support Plan (IBSP) meeting
- Seek out community resources that will best support individual needs
- Perform other duties, as needed
Qualifications:
- Master’s Degree in Social Work (MSW)
- Solid assessment and documentation skills
- Knowledge of California’s Lanterman Act and its related service system, Title 17 and/or Title 22 regulations.
- Ability to manage time and work autonomously while managing multiple priorities and meeting deadlines
