A nonprofit organization in New York is currently seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join their Assertive Community Treatment (ACT) Program. This role involves providing integrated medical and psychiatric care to individuals experiencing severe mental health symptoms, substance use challenges, or co-occurring conditions. Services are primarily field-based to meet recipients where they are.

About the Opportunity:

  • Location: Farmingdale, NY (with service in Baldwin, Nassau, and Suffolk counties)
  • Hours: Full-time or Part time, 37.5 hours per week (Monday – Friday, 9:00 AM to 5:00 PM)
  • Setting: Field-based, providing care in diverse community settings
  • Schedule: Standard weekday hours with flexible scheduling as needed
  • Travel: Required within Nassau and Suffolk counties; mileage reimbursement provided

Responsibilities:

The Registered Nurse (RN) or Licensed Practical Nurse (LPN) will be responsible for:

  • Providing direct, face-to-face treatment to ACT recipients in community settings
  • Conducting physical assessments, administering medications, and promoting adherence to treatment plans
  • Implementing wellness and self-management programs for chronic conditions such as diabetes, hypertension, and heart disease
  • Collaborating with psychiatrists, team leaders, and other ACT staff to monitor recipients’ health conditions and coordinate care
  • Facilitating referrals to external healthcare providers and overseeing pharmacological and laboratory services
  • Maintaining detailed records, including assessments, progress notes, and treatment plans, ensuring compliance with HIPAA standards
  • Developing person-centered care goals, providing health education, and using evidence-based practices to reduce hospitalizations and homelessness
  • Conducting risk assessments for suicide or violence and responding to emergency situations as needed
  • Participating in ACT team meetings and assisting with staff development activities

Qualifications:

For RN Candidates:

  • Licensed as a Registered Nurse by the New York State Education Department
  • Minimum three years of post-license experience providing assessment and treatment to individuals with mental illness
  • Experience in wellness and health lifestyle programs preferred

For LPN Candidates:

  • Licensed as a Practical Nurse by the New York State Education Department
  • At least one year of post-license experience, preferably with individuals experiencing severe mental illness

Preferred for Both Roles:

  • Strong organizational and communication skills
  • Ability to work independently and collaboratively within a multidisciplinary team
  • Valid driver’s license and reliable transportation

Work Environment and Physical Demands:

  • Field-based work requiring travel within Nassau and Suffolk counties
  • Utilization of personal vehicle for client visits (mileage reimbursement provided)
  • Direct interaction with recipients in diverse community settings (e.g., homes, public spaces)
  • Participation in staff meetings at the office (Freeport or Suffolk)
