A health services organization is seeking a Behavioral Health Registered Nurse to join their team. The organization provides treatment, social and professional skill building, and temporary housing to patients with serious mental illness (SMI) who are ready for discharge from the hospital but do not have immediate access to stable housing. Patient Population: Adults (Ages 18+)
Responsibilities included but not limited to:
- Assess patients based on interviews, chart reviews, and/or referrals
- Educates and teaches patients based on diagnosis and need • Administers medications as needed

Initiates appropriate nursing interventions based upon assessment and/or clinical guidance, which may include:
Instructing patients and/or family members regarding treatment, medication, diagnostic procedures, follow-up appointments - Observing and identifying underlying/contributing factors, especially socio-economic, cultural, physical and language barriers
- Assists in obtaining and maintaining an accurate list of medications and ensuring patients’ understanding of those prescriptions
- Coordinates response to medical and psychiatric emergencies.
- Schedules appointments for nurse follow-up visits (e.g., wound care, medication education) and assists with accessing specialty services as appropriate
- Triages patients
- Performs vital sign checks
- Documents all observations, evaluations and interventions in the indicated forms or electronic medal records system.
- Participates in preventive health measures, including administration of vaccines.
- Demonstrates proficiency in the administration of nebulizer and/or metered dose inhalation therapy
- Uses the principles of harm reduction and a patient centered approach to deliver care
- Exhibits sensitivity to the needs of a culturally and linguistically diverse patient population
- Works in collaboration with other members of the team to deliver compassionate, patient-centered care.
- Participates in monthly care coordination meetings, staff meetings, and trainings
- Provides outreach to clients in different settings (e.g., homeless shelters, street) when necessary.
Qualifications & Experience:
- A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; For individuals issued a registered professional nursing license on or AFTER July 1, 2020, based on expected implementation of Chapter 502 of the Laws of 2017 and Chapter 380 of the Laws of 2018: A Bachelor’s of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department)
- A Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA)
- Experience or prior work in healthcare settings caring for people with experiences of homelessness, mental illness (including substance use disorders) or justice involvement strongly preferred. If there is no prior substantial experience, candidate should possess a strong desire to learn and care for this population
Preferred:
- Wound care experience
- Experience with naloxone (Narcan) administration and patient education desired
- Excellent written and verbal communication skills o Exhibits sensitivity to the needs of a culturally and linguistically diverse patient population
