A medical services company in New York City is looking to add a new Case Manager to their team in the Bronx. In this role, the Case Manager will be responsible for providing case management, housing support to participants and monthly reporting requirements that will ensure compliance with the parameters of the program.

About the Opportunity:

  • Start Date: ASAP
  • Assignment Length: Ongoing (Contract to Hire possibility)
  • Shift: Days
  • Schedule: Monday to Friday (alternating weekends)
  • Hours: 9am to 5pm

Responsibilities:

Housing Coordination

  • Serve as the primary liaison between tenants, building staff, and management to ensure timely completion of apartment and building repairs.
  • Maintain detailed logs of apartment and building conditions.
  • Report non-compliance or urgent housing issues to the Supportive Housing Manager.

Case Management & Support

  • Provide case management and on-site crisis intervention.
  • Develop and maintain service plans and monthly progress notes.
  • Conduct home visits to assess tenant well-being and program compliance.
  • Educate tenants on housekeeping, safety, budgeting, and landlord-tenant relations.
  • Facilitate monthly tenant meetings (CAB).
  • Escort tenants to social service agencies when needed.
  • Administer random drug screenings.
  • Report suspected child neglect or abuse.

Administrative & Reporting

  • Complete annual rent calculations and apartment inspections.
  • Maintain accurate and timely documentation of tenant interactions.
  • Update program databases monthly.
  • Ensure lease renewals are completed and filed appropriately.

Qualifications:

  • Associate’s Degree or minimum of 60 verifiable college credits completed, Bachelor’s Degree Preferred
  • Minimum of one (1) year previous relevant experience in a Mental or Behavioral Health environment required.
  • Ability to use business acumen to exercise good judgment and decision-making.
  • Ability to deal with sensitive issues with diplomacy and discretion.
  • Must be able to multi task with strong organizational skills.
  • Excellent interpersonal skills and able to communicate both verbally and in written form.
  • Commitment to cultural diversity and sensitivity.
  • Ability to maintain a professional demeanor under pressure, and operate with a keen sense of urgency and commitment to quality.
