A community-based organization in New York City is looking to fill an immediate need with the addition of a new Case Manager to their growing team. In this role, the Case Manager will be responsible for providing direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up

About to the Opportunity:

  • Setting: Field work
  • Assignment Length: Ongoing, TTP
  • Schedule: Monday to Friday
  • Hours: 9am to 5pm (35-hour work week and after hours if need be)
  • Patient Population: Adults (Ages 18+)

Responsibilities:

  • Provide direct services to clients, including intake, engagement, assessment, service plan development, referrals, advocacy, counseling, exit summaries, and follow-up
  • Manage a caseload and document client progress in APSNET and ETO databases
  • Monitor risk and revise service plans as client needs change
  • Refer clients to appropriate services (e.g., deep cleaning, home health, legal, mental/medical care)
  • Use motivational interviewing to complete assessments and service plans
  • Ensure service plans are completed within 30 days and reviewed every 6 months
  • Conduct home visits and maintain weekly telephonic communication
  • Complete initial home visits within 24–48 hours of assignment
  • Travel to client homes across the Bronx, Manhattan, Brooklyn, and Queens
  • Coordinate services with other providers (social, medical, legal, etc.)
  • Submit monthly reports and maintain accurate documentation
  • Participate in supervision and team meetings
  • Respond to emergencies using the Life360 panic alert system
Qualifications:
  • Minimum 1 year of Case Management experience
  • Bachelor's Degree in Social Work and/or a related field
  • Strong oral and written communication, time management, and organizational skills
  • Proficiency in Microsoft Office and standard business technology
  • Ability to climb stairs, lift/move up to 10 lbs, and conduct fieldwork

Desired Skills:

  • 4+ years of Case Management experience
  • Enrollment in MSW program
  • Bilingual (English and Spanish)
