A health services network in New York City is looking to add a Licensed Practical Nurse (LPN) to join their team at one of their foster homes in the Bronx as a Case Manager.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: Ongoing (Contract to Hire)
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Patient Population: Pediatrics in Foster Care
Responsibilities:
- Monitor each child’s health and medical treatment
- Follow up with the medical provider and the foster and/or biological family
- Provide support, counseling and training to foster parents in health and medical needs of children
- Coordinate internal services with outside medical agencies
- Make monthly home visits as well as specialty clinic appointments
- Maintain and update all required files and records
Qualifications:
- 1+ year of experience in an Acute, Sub-Acute, or Intensive Care role/setting
- Graduate of an accredited School of Practical Nursing
- NYS Licensed Practical Nurse (LPN) license
Desired Skills:
- Bachelor's Degree in Nursing
- Bilingual (English and Spanish)
- Experience working with the Pediatric population
- Experience with visiting Nurse Services
