Join a highly respected non-profit organization dedicated to providing impactful healthcare services to underserved communities. This role offers an incredible opportunity for a compassionate Nurse Practitioner or Physician Assistant (NP/PA) to make a meaningful difference through intake assessments at an intake center in the Bronx.
Position Highlights:
- Schedule: Monday–Friday
- Hours: 4 PM–12 AM
- Setting: Intake Center
- Position Type: Full-time, permanent
Key Responsibilities:
- Conducts comprehensive medical intake assessments for new clients
- Gathers patient histories and performs focused physical exams
- Identifies urgent clinical needs and coordinates appropriate care or referrals
- Accurately documents findings and treatment plans in real time
- Collaborates with interdisciplinary teams to ensure quality and continuity of care
Qualifications:
- Active NYS PA or FNP license
- Experienced Providers or New graduates welcome—supportive onboarding provided
- Strong communication skills and a passion for working with underserved populations
