A recognized health services organization on Long Island is currently seeking a dedicated professional to join their Health and Recovery Plan (HARP) team in Nassau County as their new Care Management Supervisor. In this role, the Care Management Supervisor will be responsible for the supervision of care management staff and the daily operations of the care management program.
About the Opportunity:
- Assignment Length: Temp-to-Perm
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Onsite at least 2 days a week
- Area: Hempstead, Nassau, and Western Suffolk
Responsibilities:
- Supervise a team of six Care Managers, each managing a caseload of 35 clients
- Provide guidance, feedback, and support to Care Managers in high-risk crisis situations
- Conduct fieldwork and client visits throughout Nassau County
- Ensure care managers are trained and equipped to handle their responsibilities effectively.
- Carry a small caseload of clients, as needed.
- Assist the Care managers with the development of effective clinical interventions and care plans
- Provide crisis intervention to clients in distress
- Participate in quality assurance and quality improvement activities
- Conduct staff meetings, trainings, and supervision
- Facilitate bi-weekly Clinical Case Review with the Care Managers
- Perform other duties as requested
Qualifications:
- 3 years of experience in Care Coordination or OMH Residential Services for Adults
- Bachelor's Degree in Social Work, Psychology, Human Services, or a related field
- Experience in a Supervisory role
- Post-graduate experience working with individuals or families who have a history of Mental Illness, Substance Use, Homelessness, or HIV/AIDS
- Excellent problem-solving, decision-making, and judgment skills
- Excellent interpersonal and time management skills
- Ability to work on an interdisciplinary team
Desired Skills:
- 2+ years of experience working with the Mentally Ill population
- Master’s Degree in Social Work
- NYS Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW)
- Bilingual (English and Creole or Spanish)
