Home Health Registered Nurse (RN)

A healthcare organization in Grand Rapids, MI, is seeking a dedicated Home Health Registered Nurse (RN) to join their team. This role involves providing high-quality, compassionate care to patients in the comfort of their homes. **About the Opportunity:** - **Hybrid or Remote:** Onsite 1x a week at the Grand Rapids office; field visits required. - **Shift:** Monday to Friday - **Schedule:** 8 AM - 5 PM - **Hours:** Full-time - **Setting:** Home visits and assisted living facilities **Responsibilities:** The **Home Health Registered Nurse (RN)** will be responsible for: - Managing a caseload of home health care patients and their families. - Providing comprehensive assessments and care plans. - Ensuring patient and staff confidentiality as per HIPAA regulations. - Maintaining accurate and complete patient documentation. - Coordinating with interdisciplinary teams to deliver quality care. - Evaluating patient progress and modifying care plans as needed. - Communicating significant health changes to the appropriate personnel. **Qualifications:** - Minimum 1 year of experience in home health, hospice, assisted living, or long-term care. - Valid RN license; BSN not required. - Proficient in basic computer skills and EMR systems. - Strong communication skills and the ability to work independently. - Current valid driver’s license and reliable transportation. - Ability to travel within the service area as needed. **Desired Skills:** - Experience with HomeCare HomeBase or OASIS is advantageous. - Familiarity with adult patient populations, particularly 50+. - Ability to handle confidential information professionally. - Previous experience in a similar healthcare setting preferred.
