We're currently seeking a dedicated Foster Care Licensed Practical Nurse (LPN) to join a reputable organization in the Bronx, NY. This position primarily involves fieldwork and offers the opportunity to make a significant impact on the lives of foster care youth through compassionate nursing care and support.
About the Opportunity:
- Hybrid or Remote: On-site required with 60-70% fieldwork
- Shift: Daytime
- Schedule: Monday to Friday, 35 hours/week (9 AM - 5 PM or 10 AM - 6 PM)
- Hours: Full-time
- Setting: Office and field-based in the Bronx
Responsibilities: The Foster Care Licensed Practical Nurse will be responsible for:
- Managing a high caseload with interactions ranging from weekly to monthly
- Providing health assessments and establishing medical homes for adolescents
- Acting as a liaison between health providers for comprehensive care
- Delivering emotional and psychological support to youth and families
- Promoting independence by educating on health conditions and self-care
Qualifications:
- 1+ years of experience as an LPN
- Graduate of an accredited practical nursing program
- NYS Licensed LPN
- CPR Certified
- Firm command of nursing principles and practices
- Excellent communication skills
- Commitment to trauma-informed care
- Ability to work collaboratively
- Bilingual Spanish required (consideration if not bilingual)
