A health services network in New York City is actively seeking a Licensed Practical Nurse (LPN) to join one of its growing long-term care facilities in Brooklyn.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: 13-weeks
- Shift: Day or Evenings or Nights
- Schedule: Full-time (5 days a week) or Part-time (3 days a week); must be available to work every other weekend (Saturday & Sunday)
- Hours: 7:30am to 3:30pm or 3:30pm to 11:30pm or 11:30pm to 7:30am
- Setting: Long-Term Care
Responsibilities:
- Provide bedside nursing care including taking and recording vital signs (BP, pulse, respiration, temperature) and monitoring intake/output
- Observe residents for changes in condition—physical, mental, behavioral—and report to the supervising RN or provider
- Administer prescribed medications (oral, injections, insulin), manage IVs, catheters (Foley), and routine basic wound care/dressing changes
- Perform treatments such as tracheostomy care and finger‑stick blood glucose monitoring
- Change sterile dressings, assist with wound checks, and support the removal or maintenance of tubes/catheters, as delegated
- Coordinate multidisciplinary strategies to address resident issues, as part of the healthcare team
- Chart all nursing care, treatments, observations, and interventions in electronic medical records (e.g., PointClickCare)
- Provide emotional support and patient/family education regarding disease processes, medications, and self-care
- Perform other duties, as needed
Qualifications:
- 6+ months of Long-Term Care experience
- Graduate of an accredited Practical Nursing program
- NYS Licensed Practical Nurse (LPN)
