A health services network on Long Island is currently seeking a new Licensed Practical Nurse (LPN) to join their growing team at one of their facilities in Nassau County.

About the Opportunity:

  • Start Date: ASAP
  • Assignment Length: Ongoing
  • Setting: Residential ICF
  • Shifts: Days, Evenings or Overnights
  • Hours: 7a-3pm, 3p-11pm or 11p-7am (5 shifts a week and every other weekend)

Responsibilities:

The Licensed Practical Nurse (LPN) will:

  • Assist in data collection on individuals within assigned caseload
  • Contribute to the development of individual’s plans and assist in identifying areas of need.
  • Assist in observation and evaluation of nursing care administered by Direct Support Professionals
  • Ensure that all required documentation is completed /processed appropriately and in a timely manner
  • Assist in maintaining effective communication with staff, physicians and Registered Nurse.
  • Review physician summary forms to ensure information is complete, accurate, and documented in the individual’s medical record
  • Ensure Emergency Room instructions and/or treatment is followed up and documented accordingly
  • Attend medical appointments when necessary and/or requested by the Supervisor
  • Assist with ensuring accuracy of MARs for use at beginning of the month
  • Ensure all individual’s self-preservation through implementation of the fire evacuation plan when participating in scheduled fire drills
  • Perform other duties, as assigned

Qualifications:

  • Graduate of an accredited Practical Nursing program
  • NYS Licensed Practical Nurse (LPN)
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills:

  • Bachelor's Degree
  • Experience working with the differently-abled population
  • Knowledge of OPWDD State and Federal regulations
