A rehabilitation services organization on Long Island is currently seeking Licensed Practical Nurses (LPN) to join one of their skilled nursing facilities in Suffolk County.
About the Opportunity:
- Setting: Skilled Nursing Facility
- Shifts: Days, Evenings, Nights
- Schedule: 4-5 shifts per week
- Hours: 7am to 3pm or 3pm or 11pm or 11p-7a (Open to double 16 hour shifts)
- Location: Yaphank, New York
Responsibilities:
- Assist residents with activities of daily living (ADLs): including bathing, dressing, grooming, toileting, and feeding.
- Monitor and report changes in residents' conditions: such as vital signs, behavior, or mobility changes to nurses or supervisors.
- Provide mobility support
- Maintain a clean and safe environment: including changing bed linens, cleaning resident areas, and ensuring equipment is sanitized.
- Document care provided: accurately record daily tasks and any observed changes in the resident's physical or mental condition.
Qualifications:
- Graduate of an accredited School of Nursing
- NYS Licensed Practical Nurse (LPN)
- Solid assessment and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
