A hospital in Rockland County is currently seeking a licensed Nurse Practitioner to join their team for an exciting opportunity at their Pride Center.
About the Opportunity:
- Schedule: Monday to Friday- Open to full or part time
- Specialty: Primary Care- Pride Center
Responsibilities:
The Nurse Practitioner (NP) will:
- Provide direct care and consultations to patients
- Serve as a consistent Care Coordinator through the continuum of care
- Assess the physical, psychological, spiritual, and social needs of the patient
- Direct patients to the next appropriate service and track their progress
- Assure that all patients receive appropriate evaluation, diagnosis, treatment, medical screening, and medical evaluation whenever indicated
- Perform other duties, as needed
Qualifications:
- NY Nurse Practitioner (NP) license

- Experience treating the LGBTQ community
