A health services system in New York is looking for a licensed Occupational Therapist (OT) to join their team throughout Queens.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm (1 hour lunch)
- Setting: Hybrid (Clinic and Field)
Responsibilities:
- Assess individuals referred by healthcare providers and implement treatment plans collaboratively
- Complete required documentation such as assessments, progress updates, treatment notes, discharge summaries, and interdisciplinary notes in accordance with organizational policies
- Deliver evaluations and therapeutic interventions at designated locations, including clinical sites, patient homes, and care facilities
- Ensure that the environment is maintained in a clean and safe condition, adhering to all safety protocols
- Participate as an active member of the interdisciplinary care team to help develop and revise individualized care plans in compliance with applicable regulations
- Record relevant participant information including evaluations, progress notes, and care planning documentation, in accordance with established procedures
- Maintain and report statistical and operational data as required
- Participate in quality assurance and performance improvement activities
- Perform other duties, as needed
- Qualifications:
- Completion of an accredited Occupational Therapy program
- Bachelor’s Degree
- Proficiency in the use of Occupational Therapy modalities and related equipment
- Capability to travel to off-site locations, including patient residences and affiliated facilities
Desired Skills:
- 1+ year of Clinical experience in Occupational Therapy
- Experience working with the Geriatric population
- Familiarity with regulations related to Elder Care services
- Experience with Electronic Medical Records systems (such as Care Compass)
