A well-established healthcare organization is seeking a compassionate, licensed Nurse Practitioner to join its Palliative Care team. Based in Brooklyn and Manhattan territories, this role involves providing care to patients in residential, inpatient, and nursing home settings.
Responsibilities:
- Providing palliative care consultation services and NP-appropriate services to patients enrolled in hospice and home care services
- Engaging in medical history-taking, physical examinations, formulating care plans, and treatment recommendations
- Using an evidence-based approach to management and use of organizational materials and policies
Qualifications:
- Active ANP, FNP, or GNP license in the State of New York
- Palliative Care, Hospice, Home Care, Oncology, and/or Geriatrics experience
