A skilled nursing and rehabilitation facility in Concord, MA, is currently seeking a Contract Physical Therapist with the possibility of conversion to a permanent role. This position offers an exciting opportunity to provide high-quality care in a 150-bed facility and become a key part of their interdisciplinary team.
About the Opportunity:
- Hybrid or Remote: On-site only
- Shift: Day shift
- Schedule: Monday to Friday with one weekend rotation
- Hours: Full-time or part-time, consistent weekly schedule
- Setting: Skilled nursing and rehabilitation facility
Responsibilities:
The Physical Therapist will be responsible for:
- Providing and directing rehabilitation care within accepted practice methods, meeting all center, state, and federal standards
- Evaluating resident needs and establishing effective treatment plans
- Coordinating supervision of treatment plans with staff and other disciplines
- Participating in resident screenings according to directed schedules
- Providing feedback on clinical and rehabilitation issues
Qualifications:
- 1+ year of experience
- Master’s degree in Physical Therapy
- Current and active Physical Therapist license in MA
- Knowledge of governmental and regulatory guidelines in healthcare settings
- Excellent communication skills
- Strong organizational skills

Desired Skills:
- 1+ year of experience in older adult rehabilitation
- Master’s degree in Physical Therapy
