A well-known homecare organization in New York is seeking a Registered Dietician to join their team. This position involves providing essential nutrition care within patients' homes across various boroughs, making a tangible difference in individuals' health and well-being.
About the Opportunity:- Hybrid or Remote: Field
- Schedule: Part-time; 9am-5pm; 3 days per week
- Setting: Homecare
- Contract Length: 13 weeks, with the potential for extension
The Registered Dietician will be responsible for:
- Conducting assessments of patients' nutritional needs
- Interviewing patients and/or families to obtain dietary history and goals
- Preparing and documenting nutritional assessments and care plans
- Monitoring and evaluating nutritional therapies and food-drug interactions
- Coordinating care through interdisciplinary team meetings and consultations
- Conducting daily home visits and utilizing public or private transport as needed
- Participating in Quality Assurance and Performance Improvement (QAPI) programs
- 1+ years of experience as a Registered Dietician
- Bachelor’s Degree or higher in Dietetics or a related field
- New York State License as a Registered Dietician
- Current registration with NYS Department of Education in Nutrition
- Certification in Basic Life Support, including CPR (AHA)
- Proficiency with Microsoft Office (Outlook, Word, Excel, PowerPoint, Visio, OneNote)
- Strong communication and organizational skills
- Ability to work independently and make informed decisions
- Attention to detail and accuracy in documentation
- Some knowledge of home care regulations
- Experience with EPIC EMR
