A New York City health services network is actively seeking a licensed Registered Nurse (RN) to join their growing Adult Homecare team in Brooklyn. Under the direction of the supervising Assistant Director of Nursing in Community Care, the Registered Nurse (RN) will be responsible for providing community-based professional nursing care to patients in group and private homes.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: 13-week contract
- Schedule: Flexible
- Hours: 9am to 5pm
- Setting: Home Care (At Home / Community Care)
- Patient Population: Adults (Ages 18+)
- Patient Volume: 5 to 8 cases per day
- EMR: EPIC
Responsibilities:
- Diagnose and treat responses to actual or potential health problems via case-finding, health teaching, health counseling, etc.
- Assess, plan, implement, and evaluate the patient’s plan of care
- Identify and distinguish between physical and psycho-social signs and symptoms
- Select and perform those therapeutic measures which are essential for effective execution of the nursing regimen for assigned patients/clients
- Perform assessment of behavioral health needs, wound care, catheter insertion, medication teaching, diabetic care, patient/family education and counseling and aseptic technique
- Record and maintain nursing care plans and progress notes on patients/clients to ensure continuity of care and initiate, develop and implement revisions as needed
- Interpret and report responses of patients/clients to appropriate members of the health team and make referrals to other disciplines
- Provide teaching, guidance, and direction to allied nursing personnel in rendering patient care
- Authorize & coordinate all services, including nursing, rehab, respiratory therapy, nutritional services, equipment, professional and non-professional staffing and support services
Qualifications:
- 2 years of previous Acute Care experience
- Associate’s Degree in Nursing
- Active NYS Registered Nurse (RN) license
- BLS certification
- Experience in Adult Care, Critical Care, Home Health, and/or Medical-Surgical role/setting
- Computer savvy
- Solid assessment and critical thinking skills
Desired Skills:
- Bachelor’s Degree in Nursing
- Previous experience working with EPIC Electronic Medical Records
