A Long Island nonprofit services agency is looking to add a new, licensed Registered Nurse (RN) or licensed Practical Nurse (LPN) to join their growing team on a full-time basis at one of their residential facilities in Suffolk County.
About the Opportunity:
- Setting: Residential
- Shift: Day

Schedule: M-F 7am to 3pm OR M/W/F with Friday being 7am to 7pm
- Patient Population: 6 to 21 year old individuals with ID/DD
Responsibilities:
The Nurse will:
- Develop a person-centered plan of care that prescribes strategies and interventions
- Support discharge planning process with members of relevant teams
- Provide or coordinate the resident / family education
- Collect comprehensive data pertinent to the resident's health or the situation
- Perform all other duties as assigned
Qualifications:
- 6+ months of related Nursing experience
- Graduate of an accredited School of Nursing
- NYS Nurse (RN or LPN) license
- BLS certification
- Strong understanding of the needs of vulnerable populations including youth and young adults with Autism, Developmental Disabilities, Communication Impairments, and Medical Complexity
- Solid assessment, clinical and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- Bachelor's Degree in Nursing
- National Certification in area of specialty
