A nonprofit organization is currently seeking a licensed Registered Nurse to join their multidisciplinary FACT Team. This role focuses on delivering community-based care to adults with serious mental illness, substance use disorders, and criminal legal involvement.
About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Community-based across multiple boroughs (80% field-based, 20% office/remote)
Responsibilities:
- Providing field-based nursing care, medication administration, and assessments
- Coordinating care with prescribers, housing, hospitals, and justice partners
- Conducting phlebotomy, monitoring labs, and managing side effects
- Delivering psychoeducation and wellness counseling
- Completing timely clinical documentation
- Participating in team meetings, supervision, and on-call rotation
Qualifications:
- 3+ years of relevant experience
- Registered Nurse license (NY State)
- Phlebotomy and medication administration skills
- Experience with mental illness, substance use, or forensic populations
- Strong care coordination and communication skills
- Highly organized, self-motivated, and computer proficient
Desired Skills:
- Spanish fluency
- Experience in Community or Field Nursing
- Prior work with justice-involved or underserved populations
- Motivational interviewing and crisis management
