A Long Island nonprofit services agency is looking to add a new Licensed Practical Nurse (LPN) or licensed Registered Nurse (RN) to join their growing team at one of their residential facilities in Suffolk County.
About the Opportunity:
- Shift: Days
- Schedule: Monday to Friday
- Hours: 7am to 3:30pm
- Setting: Residential
- Patient Population: 11 to 21 year old individuals with ID/DD
Responsibilities:
- Develop a person-centered plan of care that prescribes strategies and interventions
- Support discharge planning process with members of relevant teams
- Provide or coordinate the resident / family education
- Collect comprehensive data pertinent to the resident's health or the situation
- Perform all other duties as assigned
Qualifications:
- 6+ months of related Nursing experience
- Graduate of an accredited School of Nursing
- NYS Licensed Practical Nurse (LPN) and/or licensed Registered Nurse (RN)
- BLS certification
- Strong understanding of the needs of vulnerable populations including youth and young adults with Autism, Developmental Disabilities, Communication Impairments, and Medical Complexity
- Solid assessment, clinical and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
