A health services organization in New York City is currently seeking a licensed Registered Nurse (RN) to join their growing team as a Care Manager. In this role, the Care Manager (RN) will be responsible for playing a crucial role in meeting the health needs of children and families enrolled in the Preventive Services Program.

About the Opportunity:

  • Office plus Field
  • Schedule: Monday to Friday
  • Hours: 9am to 6pm or 10am to 7pm

Responsibilities:

The Care Manager (RN) will:

  • Review healthcare reports received and coordinates follow-up as needed with the social work team and family members
  • Maintain communications with case planners concerning family’s health status and answers staff questions concerning family’s healthcare
  • Assist family members in locating community resources for health care and makes referrals for the families
  • Prepare medical summaries as requested for school placement, inpatient hospitalizations, etc.
  • Educate families about their conditions, its effects, the importance of keeping scheduled appointments and complying with their medical regimen
  • Obtain and record health data on family members for the purpose of tracking pertinent patient history
  • Conduct home visits at minimum on a monthly basis to offer nursing services, advice, counseling and instructions
  • Provide health information materials to staff and families
  • Attend mandatory agency in-service trainings and outside community trainings as relevant to the position for continued professional development
  • Perform other duties, as needed

Qualifications:

  • Bachelor's Degree in Nursing
  • NYS Registered Nurse (RN) license
  • Familiarity with MyEvolv, Promis, and Connections systems
  • Solid assessment, clinical, and documentation skills
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills:

  • Experience in working with a medically fragile population
  • 2+ years of previous experience in a Home Healthcare role/setting
