An established healthcare provider in North Wales, PA is currently seeking a dedicated RN Patient Care Manager for a Hospice setting. This role involves overseeing a dynamic clinical team and ensuring the provision of exceptional patient care.
About the Opportunity:
- Hybrid or Remote: Onsite required
- Shift: Day
- Schedule: Monday to Friday, 8:30 AM - 5:00 PM
- Hours: Full-time
- Setting: Hospice
- On-call: Every 6-7 weeks, with weekend site oversight
Responsibilities:
The RN Patient Care Manager will be responsible for:
- Directing the provision of hospice care through the Interdisciplinary Team (IDT).
- Supervising and managing RNs, LPNs, and on-call staff.
- Managing PTO requests and performance evaluations.
- Mentoring and orienting new hires.
- Conducting regular case conferencing and required audits.
Qualifications:
- 2-3 years of professional nursing experience, with hospice experience preferred.
- Associate’s degree in Nursing.
- State Certified RN license required.
- Supervisory or managerial experience (1-2 years).
- Valid Driver's license and reliable transportation.
- Strong verbal and written communication skills.
- Willingness to travel occasionally.
