A social services organization in California is currently seeking a new Service Coordinator for a promising opportunity with their growing team in Sonoma County. In this role, the Service Coordinator will be responsible for managing the delivery of services to regional center clients, including those with intensive physical or emotional needs and ensuring that the client's Individual Program Plan and its objectives are pursued.

About the Opportunity:

  • Start Date: ASAP
  • Assignment Length: Contract to Hire
  • Schedule: Monday to Friday - Hybrid
  • Hours: 7am-9:30am to 5:30pm
  • Patient Population: Pediatrics, Adults, or mixed (all ages of individuals that were diagnosed with an intellectual disorder)

Responsibilities:

  • Write concise, accurate and objective Interdisciplinary Notes regarding the current status of the client and related service coordination activities on a monthly basis
  • Communicate effectively in person, on the telephone and in writing
  • Read, interpret, and follow complex policies, procedures, laws, and regulations
  • Interact tactfully and effectively with those contacted in the course of work
  • Maintain current workload with accurate and complete documentation
  • Conduct meetings, interviews and evaluations; interpret and document data
  • Visit clients in their homes, schools, and day programs
  • Record accurately and timely on time sheet and mileage forms
  • Perform other duties, as assigned

Qualifications:

  • 3+ years of experience in a field related to Developmental Disabilities
  • Bachelor's Degree and/or Master's Degree in a field related to Developmental Disabilities
  • Knowledge of Human Growth and Development
  • Knowledge of Developmental Disabilities and Case Management functions
  • Solid assessment and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills:

  • Bilingual
  • Experience working with the IDD population
  • Specific experience and/or education in one or more of the following areas: Social Work; Rehabilitation; Special Education; Child Development; *Clinical Counseling; Genetics and Developmental Psychology
