A mission-driven, nonprofit senior care organization is seeking a licensed social worker to support short-term coverage in a skilled nursing setting. This role plays a key part in discharge planning, regulatory compliance, family meetings, and interdisciplinary collaboration with clinical staff. Key Responsibilities:
- Coordinate admissions and discharges
- Lead family and care planning meetings
- Maintain all regulatory documentation, including MDS entries
- Collaborate daily with nurses, therapy, and administration
- Provide emotional and resource support to residents and families
Qualifications:
- Active License
- Experience in long-term care or skilled nursing required
- Familiarity with regulatory requirements and Medicare processes
- Strong organizational skills and ability to work independently
- Compassionate and professional communication style
