A health services network in California is seeking a Social Worker (MSW) to join their team at one of their group homes in Los Angeles County.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: Ongoing (Contract to Hire)
- Schedule: Tuesdays & Fridays (Part-time)
- Hours: 8am to 12pm
- Patient Population: Adults and Families with mental health and substance abuse challenges
Responsibilities:
- Assist in the completion of assessments and plans for each person in the home, obtaining, developing and recording the information necessary for the completion in person or remotely and ensuring compliance with all applicable regulations and timelines
- Completes intake assessments during the referral process, reviewing current and historical records and documentation, performing direct observations and interviews with the individual and their support group to determine support needs and services required for success
- Participates in the transitional planning process working closely with the Qualified Behavior Modification Professional (QBMP), including Individual Education Plan (IEP), Individual Program Plan (IPP), Individual Behavior Support Plan (IBSP) meetings and seeks out community resources that will best support individual needs
Qualifications:

Master Social Worker in CA
Desired Skills:
- Knowledge of California’s Lanterman Act and its related service system, Title 17 and/or Title 22 regulations
- Highly organized and flexible with ability to manage time and work autonomously while managing multiple priorities and meeting deadlines
