A nonprofit organization in New Jersey is actively seeking a licensed Social Worker to join one of their growing facilities.
About the Opportunity:
- Assignment Length: Ongoing (6+ week minimum)
- Schedule: Monday to Friday
- Hours: Standard business
- Setting: Skilled Nursing Facility
Responsibilities:
- Coordinate admissions and discharges
- Lead family and care planning meetings
- Maintain all regulatory documentation, including MDS entries
- Collaborate daily with nurses, therapy, and administration
- Provide emotional and resource support to residents and families
- Perform other duties, as needed
Qualifications:
- Bachelor’s Degree
- Active NYS Licensed Social Worker
- Experience in Long-Term Care and/or Skilled Nursing setting
- Familiarity with Regulatory requirements and Medicare processes
- Solid assessment, clinical, and documentation skills
Desired Skills:
- Bachelor’s and/or Master’s Degree in Social Work
- NJ Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW)
