A community-based outpatient mental health program is seeking a Substance Abuse Specialist to join its Assertive Community Treatment team. This is a clinical role providing recovery-focused, harm-reduction services to individuals with co-occurring mental health and substance use conditions.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 35 hours/week
- Program: Assertive Community Treatment (ACT)
Responsibilities:
- Manage a caseload assigned by the Team Leader
- Conduct community-based individual and group counseling with a focus on harm reduction and recovery
- Complete all assigned field visits within the standard 7-hour service day
- Transport clients when needed (e.g., from hospitals or court appointments)
- Facilitate substance use-focused group sessions tailored to client treatment plans
- Complete initial and ongoing substance abuse assessments; adjust interventions accordingly
- Submit progress notes on the day of service
- Complete discharge planning and conduct 30, 60, and 90-day follow-ups post-discharge
- Coordinate with medical staff to ensure timely documentation post-hospital discharge
- Report any client incidents or critical events in line with agency protocol
- Perform other duties, as needed
Qualifications:
- Master’s Degree in Social Work, Mental Health Counseling, Psychology, or Rehabilitation Counseling
- At least 1 year of experience in a Clinical and/or Rehabilitation role/setting
- Experience with Mental Health illness affected populations and/or Substance Use Disorder individuals
- Solid assessment and documentation skills

- Certified Rehabilitation Counselor Certification
