A social services organization in New York City is actively seeking a dedicated and personable professional to join one of their facilities as a Wellness Coach.
About the Opportunity:
- Assignment Length: Ongoing
- Schedule: Saturday to Wednesday
- Hours: 2pm to 10pm
- Setting: 45-bed co-ed community facility
- Population: High-risk
Responsibilities:
- Provide case management and develop individual support plans
- Assist with daily living skills and crisis intervention
- Accompany residents to medical/psychiatric appointments
- Facilitate community integration activities
Qualifications:
- 2+ years of experience working with the Behavioral Health population
- Associate’s Degree
- Solid assessment and documentation skills
Desired Skills:
- Bachelor’s Degree
- Bilingual (English and Spanish)
- Experience working with the Mental Illness, Substance Use, and Homelessness populations
